Cornerstone is funded by the U.S. Department of Education
Office of Special Education Programs (OSEP) to support NH's development
of early childhood outcomes indicators for children ages Birth through

with young children and their families: Family Members, Service Providers,
and School, Organization and Agency Personnel to participate in the project
through involvement with the project subcommittees and/or joining NH's

For several years, NHDOE and NHDHHS have been aligning their child
and family-serving initiatives, building community-based Systems
of Care and Education (SOC and Ed). An interagency improvement plan,
known as the QUILT Project, formalized an infrastructure to align
and coordinate all programmatic efforts. Each project (Cornerstone
is shaded in yellow) maintains its own management and operations
structure and is represented on the CCMC, described below:
Children’s Care Management Collaborative (CCMC):The
Children’s
Care Management Collaborative (CCMC) is a state-level team
comprised of representatives from state-level child-serving agencies
and family advocacy organizations. Cornerstone Part C and B agency
staff and Family Organizations are represented on the CCMC.
Systems of Care and Education Data Management Team:The
SOC and Ed Data Management Team is an interagency team that works
to coordinate and align data systems to address data collection and
reporting needs at the community, state and federal levels.
Joint Steering Committee: The Joint Steering Committee
is comprised of members of the advisory committees to Parts C and B, The
Interagency Coordination Committee (ICC) and the State Advisory Committee
(SAC).

by the Preschool Technical Assistance Network (PTAN, Joan Izen)
and the Early Education Intervention Network (EEIN, Suzanne Iverson
and Sandy Brallier). All individuals, agencies and organizations
interested in forming and participating in NH’s
